The Chasam Sofer rules that serrated lower edges of the lung are normal anatomy (dildulei ha-re'ah) and not a problematic adhesion, though he personally refrains from ruling when wrinkling is pronounced. Surface pockmarks on the lung are not a concern even by the Rambam's stricter standard, as they appear universally due to inflation. However, when multiple factors combine — including the Ohel Moed's revi'is deficiency ruling and the presence of a kmet — one must rule strictly even at significant financial loss.
