Validity of Get Containing Only a Woman's Forgotten Illness-Name

A woman named Slavve received a get listing only that name, but a name-change during childhood illness had been entirely forgotten by all parties. The Chasam Sofer rules the get valid on multiple grounds: R' Peretz's requirement of two gittin applies only l'chatchila and not in cases of iggun; the forgotten name need not appear once recovery has occurred; women's names are more easily forgotten and R' Peretz's stringency does not apply to them; and in any event the name Slavve functions as a valid chanikah, universally accepted by all poskim.
