The Chasam Sofer responds to two questions from R. Akiva Eiger: he distinguishes the Ran's logic about adhesion formation (natural and inevitable) from chest-wall intertwining (external and contingent), explaining why the latter cannot be assumed to have occurred; and he resolves difficulties about the Rashba's application of tata'ah gavar by showing that whatever is originally in its place permanently retains its tata'ah designation, a principle Tosfos rejects.
