The Chasam Sofer addresses how a valid get can be arranged when the husband alternates between sanity and insanity, analyzing whether zechiyah can substitute for formal shelichus and how to confirm the wife's ongoing consent. He proposes a practical arrangement whereby the wife appoints a shliach kabbalah before beis din, witnesses verify the husband's lucidity at delivery, and the wife's continued desire for the divorce is confirmed twice daily, invoking hocheiach sofo al techilaso to bridge any temporal gap. In a postscript he adds that once the wife knows she will be forbidden to her husband after delivery, there is no reason to suspect she would change her mind, and on this basis he concurs in permitting.
